Overview

Thoracic back pain is pain that affects the mid-spine (the thoracic vertebrae) and the surrounding muscles, joints, and connective tissues. Because the thoracic spine supports the rib cage, irritation of spinal joints, costovertebral joints, discs, or muscles can feel sharp, dull, or tight and may worsen with twisting, reaching, or deep breathing. Nerve involvement is less common than in the low back, but it can occur and may cause pain that wraps around the chest or affects sensation. Most cases improve with conservative care, but certain symptoms require prompt evaluation to rule out serious conditions.

Symptoms

Patients often feel aching, stiffness, or soreness in the mid-back between the shoulder blades or along one side of the spine. Pain may worsen with posture changes, prolonged sitting, bending, twisting, lifting, or taking deep breaths, and it can sometimes be reproduced by pressing on specific spots. Some people notice muscle tightness and reduced range of motion, while others may develop pain that radiates around the rib area. In more concerning cases, there may be numbness, weakness, or balance problems.

Causes

Thoracic back pain commonly develops from mechanical strain or overuse, such as prolonged sitting, poor ergonomics, or sudden activity that overloads the mid-back muscles and joints. Degenerative changes in the spinal joints or discs can also irritate pain-sensitive structures, particularly with age and repetitive stress. Less commonly, the pain can be driven by inflammation, trauma, or nerve irritation when a disc, joint, or other structure affects nearby nerves.

Risk Factors

Risk increases with occupations or activities that involve long periods of sitting, repetitive twisting, or heavy lifting without adequate conditioning. Weak core and upper-back endurance, poor posture (including forward head and rounded shoulders), and infrequent stretching can contribute to muscle imbalance and joint irritation. Smoking, excess body weight, and reduced overall physical activity can impair tissue health and recovery. A history of osteoporosis, cancer, significant trauma, or chronic inflammatory disease can raise concern for non-mechanical causes.

Prevention

You can reduce recurrence by keeping a regular movement routine that includes gentle mobility for the thoracic spine and strengthening for the upper back and core. Aim for ergonomic setup with supportive seating, frequent position changes, and a neutral posture while working or driving. Gradually build tolerance for lifting and sports activities, and avoid sudden spikes in workload. Maintaining a healthy weight, not smoking, and sleeping on a supportive surface can also help tissues recover more effectively.

How the Diagnosis Is Evaluated

Clinicians usually start with a focused history that clarifies onset, activity triggers, severity, associated symptoms (such as numbness or weakness), and any red flags like fever or major trauma. A physical exam typically checks posture, range of motion, tenderness patterns, muscle strength and reflexes, and sensory changes to assess for nerve involvement. Imaging is not routinely needed for uncomplicated cases, but X-ray, MRI, or CT may be considered when symptoms are severe, progressive, follow significant injury, or suggest infection, fracture, or cancer. The evaluation is tailored to the patient's risk factors and neurologic findings.

Nonsurgical Treatment Options

Most thoracic back pain improves with conservative care focused on reducing irritation and restoring movement. Activity modification for a short period, along with heat or ice and over-the-counter pain relievers such as acetaminophen or nonsteroidal anti-inflammatory drugs (if safe for you), can help manage symptoms. Physical therapy is often central and may include thoracic mobility work, posture and scapular strengthening, core stabilization, and graded return to activity. For painful muscle trigger points, trigger point injections may be considered in selected cases. If pain appears driven by facet or costovertebral joint inflammation, a clinician may consider corticosteroid injections to reduce inflammation and improve function. For persistent pain that does not respond to standard therapy, some specialists may discuss options like platelet rich plasma (PRP) or other biologic approaches on a case-by-case basis, though these are not first-line for typical thoracic mechanical pain.

When to Seek Medical Attention

Seek prompt medical attention if you have severe or worsening pain after trauma, especially if you cannot function normally or cannot move the spine as usual. Go urgently if you develop progressive weakness, numbness, trouble with walking or balance, or any bowel or bladder changes. You should also get medical care quickly for fever, unexplained weight loss, a history of cancer, night pain that is not positional, or severe pain that does not improve with rest. If the discomfort is accompanied by chest pressure, shortness of breath, fainting, or symptoms concerning for a heart or lung problem, treat it as an emergency.
