Exosomes are extracellular vesicles — tiny membrane-bound particles secreted by cells — that carry a payload of growth factors, cytokines, and signaling molecules. In laboratory and early clinical research, exosomes have been shown to influence cell behavior, modulate inflammatory signaling, and may support tissue repair processes. Commercial exosome products used in orthopedics are typically derived from carefully screened donor tissue and processed to isolate these vesicles.

Mechanism

How Exosome Therapy May Work

Exosomes are administered via targeted injection, typically under imaging guidance. Because they are acellular — they do not contain whole cells — they interact with local tissue through the signaling molecules they carry rather than by engrafting or differentiating. Early research suggests they may help reduce inflammatory signaling and support cellular repair responses in injured or degenerative tissue. Most current evidence comes from preclinical and small clinical studies; robust randomized controlled trial data in orthopedics is still emerging.

Evidence and Limitations

What the Research Shows

Exosome research is an active and rapidly developing area. Laboratory studies and early human data are encouraging, particularly for conditions involving tendon injury, cartilage degeneration, and joint inflammation. However, exosome therapy for most orthopedic indications is still considered investigational. There are currently no FDA-approved exosome products for orthopedic use, and high-quality randomized controlled trials are limited. The FDA has expressed significant concern about certain commercial exosome products marketed with unsubstantiated claims of treating serious conditions. The FDA does not currently approve any exosome product for orthopedic use, and has taken enforcement action against products making unsupported treatment claims.
